Your personal and patient information is only shared with people at Pioneer Provider Network as well as partner organizations who need this information to respond to your requests or assist you medically or personally, or evaluate the service you receive at PPN. Information submitted through Pioneer Provider Network online forms may be collected to ensure technical functionality. It will also be utilized to address any inappropriate use of our website. We do not save your personal information for use for other purposes, nor do we provide it or attempt to sell it to other organizations.

Where the information is collected

We collect information about you at several places throughout the site. You will be informed on how this information is collected at that time. Our Relationship with Third Parties
